Eligibility

Eligibility for this service is based on the ability to pay, this is not a free service. However, on a case by case basis, Compassion North America may provide this service to an individual on a compassionate basis. Depending on the payer type, payment may be required before or after the service is delivered.

Individual payer.

Individuals can pay for this service on an out-of-Pocket basis or through a third party payer.

Out-of-Pocket payment.

Out-of-Pocket payment can be made in cash, using a credit card such as Visa, Master Card, American express etc., and other electronic payment methods such as paypal, zelle, and visa direct. Out-of-Pocket payment is required to be completed before the scheduled trip.

Third party payer.

If your trip would be paid for by a third party payer, such as any private insurance, Medicare, Medicaid, or an approved business, you are not required to pay for this service before your trip. However, you would be required to complete and sign the required paper-work, which is required by your third party-payer to pay for this service. A transportation request form, which collects the required information, would be presented to you to sign before or after your trip. This provision may be subject to the existence of a valid contract, MOU, or approval, between Compassion North America and the third party payer to provide this service, as applicable. At the time of booking your trip, you will be informed about your eligibility to receive this service, based on your submitted payer.

VA/Medicare/Medicaid

If your third party payer is Veteran Affairs, Medicare, or Medicaid, at a minimum at least one of the following unmet needs must be present:

• You do not have a valid driver's licence.
• There is no working vehicle in the household.
• You are unable to travel or wait for service alone.
• You have a physical, mental, cognitive or developmental disability.
• You are currently receiving Medicare, Medicaid, or VA benefits.

In addition to the criteria above, this service can only be provided for: transportation to and from Doctor or dental appointments, transportation to medical examination and treatment, and any other benefit clearly stated by the third party payer, which is not mentioned here. This service will supply the most appropriate means of transportation for each client.

This is to inform individuals, submitting the names of third party payers for this service, that you are responsible to pay for any trip, which your third party payer refuses to cover or pay for. It is also your responsibility to ensure that you are informed about any minimum or maximum limits placed on the use of transportation services by your insurer or third party payer. Please monitor your use of this service accordingly, to ensure that you comply with such provisions.
